This doesn't have to be elaborate. My proposal:

  • On day 1, the player has almost no blood.
  • A note is left explaining some mechanics.
  • The exit to the ladder is blocked off.
  • For the first N days, blood bags spawn in the kitchen, making up for the player's lack of thralls.

Consider building a zone 1 that is a super good encapsulation of the mechanics, as a stretch goal.
